Thank you for your interest in volunteering and providing a foster home with our rescue organization. Please complete the questions below to help us understand which activities and foster dogs would be best suited for you. If an item is not applicable do not leave blank, please enter N/A. Please feel free to elaborate or clarify any item. You can use the comments section at the end for anything else you would like to add.

The information you provide on this form will be used solely by NCBCRA for internal use and will not be shared with anyone outside of our organization. By completing this form you authorize us to use and share this information in this manner.

Have you discussed fostering with all household members and do you have everyone’s support?*
Any other regular visitors who will be in contact with foster dogs?*

Do we have your permission to contact your landlord to verify you can have a dog?*
Do you have a secure fenced yard?*
Do you have a secure kennel run?*
Do you crate your dogs when you leave the house or at night for sleeping?*

Are all current household dogs spayed or neutered?*
Are all current household dogs up to date on vaccinations (rabies, DHPP, bordetella)?*
Are all current household dogs accepting of new dogs into the home?*
Any livestock a foster dog could be in contact with?*

Dog Handing & Fostering Experience

Have you ever owned or had previous experience with border collies?*
Have you fostered dogs or volunteered for another rescue group?*
Do you have special skills or training (trainer, vet, groomer, behaviorist etc.)*
Do you have confinement available to isolate a dog for health reasons?*
Will you foster a dog with kennel cough?*
Will you foster a dog that is injured?*
Will you foster puppies?*
Will you foster a dog that has behavior issues?*
Will you foster a dog that is dog-aggressive?*
Will you foster a dog that is not housebroken?*
Have you lost a pet (not through death)?*
Would you allow a dog to ride in the bed of a truck?*
Have you ever had a dog die due to disease?*
Do you have any restrictions on the size, age, or sex of the type of dog you will foster?*

Fostering requires a large commitment and often a bond will develop that makes it difficult to give up the dog to its new home. Are you prepared for this commitment and for letting go of the foster dog?*
When bringing a new dog into your home there is risk of items being damaged, however we will not be financially responsible for personal property damage caused by your foster dog. Do you accept the possibility of damage to personal property from your foster dog and not being compensated for it?*
We pay for your foster's dog food and for veterinary care we have authorized. Except for emergency care, please do not obtain non-authorized veterinary care. Do you accept being financially responsible for non-emergency vet care you obtain for your foster dog that NCBCRA had not authorized?*
